Gall bladder pain: Are you sure it is your gall bladder? After you eat a greasy meal, are you having right upper abdominal pains about an hour afterwards? If so, better see your PCP for u/s of your gall bladder. If not, better go see him anyhow for work up. ...Read more

Chronic pneumonia: thats something you need more evaluation. Although it is likely that you had 3 pneumonias, you need to think and evaluate the possibility of atypical/chronic pneumonias or pulmonary fibrosis. If no chest X-ray infiltrate documented, and if u smoke likely that it is chronic bronchitis. Either way you need evaluation. Just a thought: but the appearance of the bite (eg bruising) is NOT typical of a mosquito bite and Springfield Mass NOT a place with any REPORTED mosquito borne illness thus far this season ( I am in Massachusetts) Sounds more like a SPIDER Bite with a reaction to the toxin .....some pretty "powerful" spiders exist in Western MA. Could still be GB: When you say the gallbladder is normal, how was it evaluated? You can have gallbladder disease even though it looks normal on ultrasound. You also could have a functional or inflammatory bowel disease. A G.I. doc can help sort those out. ...Read more

You're correct about probably too early for Lyme Antibodies: I'd run Western Blot, No ElISA, check CD57, Complement C4 , EBV and early antigen EBV, HHV6. Highly suspect for Lyme and probably co-infections : Babesia, Bartonella, Rickettsia, Erlichia, Rocky Mountain Spotted Fever. If Lyme, even though no bands show positive yet, a low CD57 along with a re-activated EBV has been a common pattern I've seen of late. ...Read more

Pneumonia: Need more info, particularly the age of these kids. Generally they should be seen and examined and if abnormalities by exam then cxr is a reasonable follow-up procedure.
