It is possible: Herniated discs in the thoracic region can cause thoracic radiculopathy -- pain that radiates into the chest in the distribution of the nerve from the spine at the level of compression from the disc herniation. This could follow an "intercostal" type pattern.

Many things: These symptoms are very nonspecific, and could be due to any number of conditions. A general medical workup should look at thyroid levels, vitamin levels, electrolytes including calcium and magnesium. Further studies for the numbness and muscle pain could include what are called nerve conduction studies.
