Depends: It sounds like you are in the midst of changing your pain med regimen. It might be that the previous pain medicine you were on had faster onset characteristics. Also, extended release is always much slower and steady in release than any instant or short acting release formulation of a pain medicine. This may take a while for you to adjust. Not big risk: Ultram increases serotonin like some antidepressants so it could be a trigger for mania, but if you're on lithium the risk of this is low. Almost all psych drugs are listed as "CNS depressants" meaning they can make you sleepy or reduce brain function in some people. As is Ultram.
