Immunoglobulin M (Overview)

There are 5 classes of immune proteins: igg, iga, igm, ige and igd. Igm fights infection. If you produce too little then infections may occur and you have hypogammaglobulinemia. If you have too much you may have a gammopathy.

Several causes: Multiple myeloma, early infections, rheumatoid arthritis and some kinds of kidney disease can cause this. The likely causes can vary with the age of the person. You need to discuss this with your physician.

Depends: If IgM is elevated but IgA & igg are decreased then you have a significant immunodeficiency called hyper IgM syndrome. Other causes of elevated IgM include monoclonal gammopathy and multiple myeloma. Never ignore the possibility of a lab error - the next test to do is to repeat the IgM level.

If monoclonal (of one kind) raises the possibility of waldenstrom's macroglobulinemia - a malignancy affecting lymphocytes or plasma cells that produce this abnormal IgM protein causing symptoms of hyperviscosity - confusion, shortness of breath, stroke, kidney damage etc.

Is it normal to have borderline low immunoglobulin m (igm) levels? Do they fluctuate that much? ...Or nothing to worry about? Doc wasn't worried.

Don't worry: It may fluctuate but I won't be concerned with an isolated low igm, let alone a borderline low one. The immunoglobulin levels, like any other products produced by the body, do fluctuate a little with time but usually within a narrow range in a healthy person. Some levels may elevate in the presence of infections or allergen exposure.

Initial vs long term: Generally we see IgM as the initial antibody that we make in response to an infection. Igg is the antibody that provides long-term protection.

High IgM: This is typically normal if seen on any one spot check. If it is high repeatedly on two or more checks, a test called " immunofixation" should be ordered to be sure it is not coming from only one population of blood cells; rarely, this could be due to a monoclonal gammopathy (a pre-cancerous condition). Or even rarer, a blood cell cancer called multiple myeloma.

What does it mean if your immunoglobulin m is low and your hemoglobin alc is high?

Unrelated: They are unrelated. Low IgM is usually not clinically significant (depending on how low of course). Talk to an allergist immunologist to figure this out. A high A1c would suggest diabetes. Talk to your primary care doctor.

What if your immunoglobulin m is elevated? Does it mean an infection?

Depends: Igm & ige are the first antibodies to rise in the face of infection. As the body refines its immune response IgA & igg antibodies specific for the infecting agent are produced while specific IgM levels fall. Rarely does the IgM response to an infection raise the total amount of igm. Exceptions are congenital infections. High total IgM in adults suggest autoimmune disease or malignancy.

See below: All laboratory results need to be interpreted in the clinical context and the doctor who ordered the tests is usually in the best position to do that. Having said that, the findings suggest auto-immune red cell loss. It would be prudent to see your doctor who ordered these tests.

Can vaccinations administered before immunoglobulin test affect outcome specifically for immunoglobulin m levels?

Live viruses maybe: Live virus vaccines may affect the igm levels. It takes 10-14 days for vaccines to stimulate an antibody response. If labs are drawn within a few days the levels should not be that affected. Best advice is to wait for 3-4 weeks after a vaccine to have immunoglobulin test drawn. If not then let your doc know when the labs were drawn. Really not a big worry.

I have a very low igg3 and igg4, along with a very low immunoglobulin m and a high serum amyloid a - what does this mean?
