Not really: Reflux results from ring of muscle at lower end of esophagus relaxing & allowing acidic food to reflux up esophagus. Fried/greasy food, spicy food, acidic food, caffeine, alcohol, nicotine, carbonated drinks, meds like calcium channel blockers/nsaids etc can all exacerbate reflux. No: No bread of any kind is preferable but whole grain is much better than refined flour white bread.

Not quite!: Avoid sugar and starch. Look at the total carb content and subtract fiber for "the digestible" carb intake. Fiber is a carbohydrate, but not digested, so it does not count. Sugar alcohols are supposed to be slowly digested, but there is some disagreement here. So to be safe-total carbs minus fiber, equals your "net" carbs!

I think you are: Asking about a low carb diet. Please check w/professional to make sure this is a good food plan for you. Meat and diary are high protein food sources. Complex carbs such as whole grains and vegetables break down more slowly in the body and are better for you than simple carbs from refined grains in bread, muffins, cookies, etc. Please look up glycemic index and eat low on that. With appropriate food intake and exercise you should gradually lower your blood sugar level to a point where your diabetes is no more. We're all Prediabeti: As we grow older, we all tend to become Insulin resistant to some degree or other. Consequently, our sugars tend to rise, along with our insulin. Insulin is the hormone that causes sugar to enter our cells, where the excess is stored as fat. I recommend a low carb diet to everyone, and also recommend that just about everyone over 30 will benefit from metformin (an Insulin receptor sensitizer).
