It sounds like your are grinding of your teeth in your sleep resulting in TMJ Syndrom. You may want to consider an over-the-counter mouth guards and see how that will works for you. If that works then you can go see your dentist to make one for you. You may use a mild muscle relaxant at bedtime (from your PCP). Use ibuprofen for pain.

Possibly sinus: Waking with a headache centered on the forehead could indicate a sinus issue. When asleep, you are laying flat, and the sinuses do not drain as well as they do when you are upright. It: It is unclear on whether your otc decongestant is a nasal spray or pill. If it is a spray, I agree with dr. Funari that useage beyond 3-5 days can lead to rebound congestion (rhinitis medicamentosa) and actually worsen your condition. If your nasal decongestant is in pill form (i.e., Pseudoephedrine containing) this will not create that type of condition. I also can not be sure where your tenderness is, but a common cause of nasal tenderness on the inside of the lower 1/3 of the nose (nostril area) is vestibulitis, or inflammation around a nasal hair follicle or oil gland.
