Comminuted fracture: Unfortunately, bone healing takes a minimum of 4-6 weeks. Staying compliant with non-weightbearing orders until then and eating a healthy diet, not smoking and (my preference) not taking NSAIDs (as there is evidence that they inhibit healing) are all important. Tight boot: Anytime you have a foot injury and use a boot, make sure the bindings are not too tight. The bindings can put excessive pressure on the fracture or injury. One of the well known risks of talar fractures is aseptic necrosis (the bone literally rots) because of a compromised blood flow to the bone after the injury. If it is not displaced stage 1 consider an injection of either platelet rich plasma, or amniotic membrane and fluid
if stage 2 + then consider ankle arthroscopy, with microfracture of the lesion, adding in prp or amniotic membrane and fluid or both

Difficult: This is a tough problem to treat (there are no tests that I know of - it's just a tough problem to treat) and ultimately it may come down to trying to live/deal with the pain as much as possible vs more drastic surgery such as an ankle fusion. Effusion: It sound like you have an effusion in your ankle. Previous trauma? Swelling in the joint or in the surrounding tissues of your ankle? A painless swollen monoarthritis should be investigated w/full exam to see if you have any infection rheumatological sx or diabetes, may progress to polyarthritis.
