Punctured wound: Development of an infection is greatest concern and will depend on how deep the nail got. If shallow, rate of infection should be almost minimal. If deep, surface cleaning make not be enough to prevent bacterial infection. No: The skin must be punctured completely to force the clostridium tetani bacteria through to underlying tissues order to cause tetanus. A scrape won't introduce it, as well.
