Probably not: Concussions can result from what can seem like a mild impact to the head, even if there are no symptoms immediately. Head injury followed by any headaches, nausea, vomiting, dizziness, or other neurologic symptoms needs immediate attention but longer term less dramatic symptoms such as trouble with mood, concentration, sleep can also be the result even with a normal ct or mri.

Exam needed: The original lump was most likely a blood collection (hematoma)under the skin of the scalp. It receded but now the new swelling could indicate rebleeding, infection, new ingrown hair or cyst. Since it's painful or tender, it should be evaluated and treated by a physician. Apply warm moist compresses until you are examined.

Head trauma: Even after trivial head trauma it is not unusual to experience headaches or scalp tenderness for a week or two. However if headaches persist or increase in intensity and frequency, or if there is accompaning nausea, dizziness, drowsiness, visual disturbances get evaluated for brain injury or bleeding.

Head bump: The bump probably caused a bit of bleeding deep next to the skull. It can calcify/scar the area. OR, you might have a small foreign body, tiny piece of rock/concrete, that embedded and caused a local inflammatory reaction. If it doesn't bother you (hurt) I would leave it alone for now. It will probably slowly go away. Of course, a surgeon would invite you into an encounter with a scalpel ;)

Concussion: Your concussion occurred 4 weeks ago. Had you a brain injury you would have experienced more symptoms than just a "tender bump", but if you are concerned, perhaps your doctor could get a skull X-ray.

Bruise: Bruise may hurt for a week or so as long as you don't have any post concussive symptoms you should steadily improve. Tenderness to touch is common.

Outside or inside: If you have a concussion, it should be minor, and you should not worry. A headache may be caused by many things. You have a bump on your scalp? That can be the source of your pain. Perhaps it is merely an external injury. Give it some time and let us know if there are any unexpected problems.

Contusion: This is most likely soft tissue bruising (contusion) or a collection of blood under the skin (hematoma). It is common to get these after a head injury. They should settle on their own over a few months. There is nothing to be concerned about.

Unlikely: While it usually takes quite a significant blow to head to cause a subdural hematoma, in patients on blood thinners or with certain medical conditions minor trauma could cause significant bleeding. You sound like you have a bruise, but with any loss of consciousness, persistent headache, blurred vision or dizziness, see you doctor.

Make sure: Do you have any confusion, memory lapse or other cognitive issues? If you are at all concerned about concussion, it would be most appropriate to see your doc. Your other symptoms are consistent with some bruising and jarring of the muscles.

See your Doctor: If you are still having pain after a week, it will be a good idea to evaluate with your physician. In my opinion, any pain lasting more than a week should be evaluated by a doctor.

Not a big surprise: If you have no other symptoms then concussion is not really at issue. Take a couple days, try tylenol (acetaminophen) and a cold pack, slow down a little and give your head a break. Sounds like it has been taking a beating.
