"Severe" sounds bad: A woman with new "severe pelvic pain" should call her primary doctor or Ob-Gyn doctor right away. Possible causes include infection, obstruction, twisted ovary, cyst or tumor problem, pregnancy problem, etc. If she has no personal doctor, she has to go where she will be examined. Usually, that would be the Emergency Room. One can call 911 for an ambulance if there is no safe way to get to the E.R. ...Read more

Get checked..: It is difficult to tell with just your question...Specific location, timing/relation to activities etc..Walking/sleeping/during relation etc...Can help tease out cause. Infection such as pelvic infllammatory disease can certainly do so, as well as ovarian cysts, salpingitis (infection of your tubes) etc..Since it has been 4 months, i would consult doc..Likely need exam and ultrasound.
