Unlikely: 3 mm is quite small. If it is unchanged in over a year, symmetrical, and fairly uniform in color it almost certainly a benign mole (nevus). Melanomas usually change in color or have color variation, grow in size, and have asymmetrical borders. ...Read more

We all heal differently, and if you had a full-thickness biopsy that was left to heal itself, it has likely scarred in.

Any time you have a mole that "changes" its time to have it evaluated and more than likely removed for analysis. It is not uncommon for them to be between the toes, mainly because of the potential for the toes to rub against each other.
