Tough case: Chronic pelvic pain is one of the more difficult diagnoses to pin down. If it is cyclical (worse with periods) then consider endometriosis as a likely possibility. You might need a laparoscopy to make the diagnosis, as ultrasound is often not helpful. Other possibilities are urinary tract, bowel, or musculoskeletal related pain. Any family member with history of ovarian or breast cancer . Any history of gene mutation like brca?
