Depends. : Pain shooting down the spine, into the legs or buttocks, typically indicates an irritation of the spinal cord in the neck (for which there are several possible causes). Pain going into the shoulder blade area, especially if it's mostly on one side, is more likely muscular or similar. If the former, you should see a neurologist. Shooting pain: Into the legs while lying flat is very likely due to compression of the spinal cord or nerves in the back by some structure like disk herniation, infection or less commonly a tumor. See your doctor for a physical exam and an mri. ...Read more

Pnched nerve: could be in a muscle in you buttocks, such as the periformis muscle which is known to pinch the sciatic nerve and cause sciatica, but can also be coming from a disk in your back. Instability due to disk disease can cause muscle spasms in the buttocks, so I would look into getting an MRI of your back if you can ...Read more

Nerve irritation: This is a sign, of irritation of nerve or nerves, coming out of your spine, providing sensation to the skin and various parts of you described. Sciatica: I'm sorry to hear about your past injuries but back problems seem to have a way of returning. When pain 'shoots down the leg' it is a sign of a pinched sciatic nerve in the lower lumbar spine. Sciatica may occur after a back fusion due to scar tissue at the site or a bulging disc --1 level below the fusion. L5-S1 : L5-s1 disks can cause severe sciatic leg pain. I would recommend consultation with a spine surgeon if you have failed 3 steroid injections. Leg pain secondary to herniated disk usually responds well to surgery. There are many surgical options including traditional and minimally invasive surgery. I would recommend looking for a surgeon who is experienced in endosocpic minimally invasive surgery. Endoscopic treatment is not always an option, but it is worth exploring due to less tissue damage, quicker discharge and recovery and less post operative pain.
