Depends. : Pain shooting down the spine, into the legs or buttocks, typically indicates an irritation of the spinal cord in the neck (for which there are several possible causes). Pain going into the shoulder blade area, especially if it's mostly on one side, is more likely muscular or similar. If the former, you should see a neurologist. Pnched nerve: could be in a muscle in you buttocks, such as the periformis muscle which is known to pinch the sciatic nerve and cause sciatica, but can also be coming from a disk in your back. Instability due to disk disease can cause muscle spasms in the buttocks, so I would look into getting an MRI of your back if you can ...Read more

Nerve irritation: This is a sign, of irritation of nerve or nerves, coming out of your spine, providing sensation to the skin and various parts of you described. L5-S1 : L5-s1 disks can cause severe sciatic leg pain. I would recommend consultation with a spine surgeon if you have failed 3 steroid injections. Leg pain secondary to herniated disk usually responds well to surgery. There are many surgical options including traditional and minimally invasive surgery. I would recommend looking for a surgeon who is experienced in endosocpic minimally invasive surgery. Endoscopic treatment is not always an option, but it is worth exploring due to less tissue damage, quicker discharge and recovery and less post operative pain. When pain 'shoots down the leg' it is a sign of a pinched sciatic nerve in the lower lumbar spine. Sciatica may occur after a back fusion due to scar tissue at the site or a bulging disc --1 level below the fusion. You need imaging studies (ct, ct myelogram or mri) to check for s1 nerve impingement. ...Read more

Neck strain: Try Naproxen twice daily and Tylenol (acetaminophen) four times daily for a couple of days. If not improving, find an inexpensive chiropractor and go for only a few visits. If no improvement after that, see your regular doctor. If there was no associated injury, l doubt it's disc related and mist likely a severe muscle spasm pinching a nerve. Consider emg: I would recommend emg/ncv to evaluate for carpal tunnel syndrome, ulnar neuropathy. Your problem may ultimately be coming from your neck. I good hand surgeon should be able to order the emg/ncv and perform a thorough history and physical exam and give you a more precise diagnosis of your problem. ...Read more

Seek consult: I would recommend you see either an orthopedic surgeon or a neurosurgeon as you may have a problem with a disc in your neck. I sounds suspicious for a "pinched nerve" or herniated cervical disc. A physical examination and possible a MRI would be helpful. One would have to: assume that this has something to do with your sleeping position. My guess is you sleep in one position (on your back?) most of the time, and, due to inactivity, certain tendons/ligaments/muscles have stiffened up a bit and need to stretch out once you arise in the morning. You might try to change positions more while sleeping (this is difficult!) or incorporate stretching in your rising routine:) ...Read more
