Testalgia: Torsion of the testis is indeed an emergency. If you really feel that this is a possibility, get examined at once in an er. I might add that "slight" pain and "slight swelling" are usually not indicative of torsion. More often this is frome epididymtis, which should also be evaluated by a physician but is usually traeated with antibiotics. ...Read more

? appendix testis: If pain is confined to tiny tender swelling on upper end of scrotum. Epididymitis or epididymal cyst could all produce theses symptoms. Suggest you see a urologist, who may well advise an ultrasound exam. Epididymitis means antibiotics & rest, epidymal cyst -> observation or excision depending upon severity of symptoms, similar for rorsion of appendix testis. ...Read more

Likely not serious: Achy, non-severe pain in testicle or spermatic cord is common at your age. Probably condition known as "blue balls" or "lovers' nuts". Often caused by erotic reactions without release. This symptom can occur with a varicocele, whih feels like a "bag of worms" in scrotum when standing & goes when lying down. See doc: It depend on the severity of the pain .if the pain is severe need to go to the ER to rule out torsion of the testicle . If the pain is mild then you might have some straining to the groin muscle that could cause pain and could lead to a hernia.
