28wks pregnant with hypothyroidism.Low = haemoglobin, platelets, RBC, haematocrit &lymphocytes. High MCH, MCV quite high. Anaemia? or possible cause?
My liver enzymes have fluctuated between normal and mildly elevated over the past 2 yrs. The alt, ggtp, alkaline phos, bili, includiing the mcv. I also have hypothyroid disease and take armour thyroid. Should i be concerned?

Multifactorial: There a several potential causes for the syncope in this lady. Of concern is that the episode on August 4th occurred without warning. This could be arrhythmic in origin. Although a Holter has been done the patient may need an Event Monitor and if this is negative an implantable monitor may be needed. We need to make sure she has normal LV function by Echo etc as she does have a history of PVC's. Vasodepressor syndrome needs to be also considered as she does have some features compatible with this. TTT would be reasonable. Yes, but rare: It has been described, but rarely. Also you can have an abnormality of thyroid binding globulin that can fool some, but not all, thyroid tests. Sometimes thyroid tests point in opposite directions for a period of time due to thyroid inflammation (thyroiditis). No one thing: No one thing would cause both of the conditions listed. However, in women, hypothyroidism can cause long heavy periods, which, over time, could lead to iron deficiency. Too many platelets is a common finding in iron deficiency anemia, and so it could be indirectly related to the hypothyroidism. Modest dose: That is a modest dose for an adult with hypothyroidism. Doses vary from patient to patient, and thus so does the need to check dose.
