Hypospadias (Overview)

Hypospadias is an abnormality where the urethral meatus (urinary opening) is located on underside of the penis. Location can be anywhere in mid-line from underside of glans (most common), along shaft, scrotum or perineum. Foreskin is always hooded & incomplete if meatus at or proximal to coronal sulcus. Chordee, ventral curvature, commonly present & always if meatus is very proximal.

Depends on severity: Hypospadias occurs with very variable severity. Some are so minor they can be corrected with a modified circumcision which should be inexpensive $3 - 500.00. Severe scrotal hypospadias with severe chordee may require staged surgery including grafts each costing $2000.00+. Most are inbetween with opening on shaft should be between $1 - 2000.00. 15%: The overall risk of hypospadias recurring in a child of a man with hypospadias is roughly 15%. Thus, as dr mesrioban notes it is a rare occurrence. Yet, as dr. History & exam: Will ask about family history, about 15% of males with hypospadias have relative with similar condition. Will ask how you urinate, if penis is bent with or without erection & sexual history. Should perform complete physical examination including chest, abdomen & genitalia (penis, scrotum & testicles. First uroflowmetry: Urologist 1st needs to determine whether or not urethral stricture is present (common complication of hypospadias repair) unless you have full urinary stream. Stricture causes slow &/or narrow stream & ejaculation problems. Then may suggest cystoscopy to inspect entire urethra including ints-urethral ejaculatory duct openings. Usually 1 or 2 stage: Operation depending upon severity of hypospadias. 3rd or 4th operation might be required in case of complications. Would suggest second opinion from an experienced pediatric urologist if problem hasn't been fixed after 3 operations. Would not be criticising original surgeon, but an other opinion could be reassuring for you. ...Read more

Yes it could: Either if untreated or following surgery. Stream will be directed downward if untreated. Also could be very narrow from associated meatal stenosis (small opening) or spraying if meatus is wide or gaping. Both can also occur after surgical repair, i, e, arrow stream from metal stenosis (common complication of surgical repair) or urethral stricture.
