What is a hydrocele?

Fluid in the scrotum: A hydrocele is a collection of fluid that accumulates between two layers of tissue called peritneum that surrounds the testis and spermatic cord. It results in swelling of the involved side of the scrotum and is sometimes painful. Causes vary from completely benign to more serious. It is thus very important that any new scrotal swelling be brought to your doctor's attention for further evaluation.

Not usually: Hydrocele which is fluid around the testicle does not usually cause symptoms unless it arises suddenly which may cause discomfort from stretching of the scrotum.

Are the symptoms of hydrocele bad?

No: The hydrocoels I see on a regular basis are in infants that seem unbothered by the process. Occasionaly parents bring in a baby boy whose been fussy & realize the scrotum seems enlarged. They assume the enlargement caused the fussiness, it is the opposite. A fussy baby can drive a little fluid into a communicating hydrocoel but on exam the structure is not tender. Most resolve without rx.

Hydrocele: Not necessarily. If the change is progressive in nature then I would report it to your pediatrician. Keep all post-op follow-up appointments as recommended

Testis pain: At your age, torsion of the testicle has to be ruled. You could be having intermittent torsion. A Doppler scrotal U/S should help make the diagnosis. If you can't be seen by your doctor ASAP, go to the ER.

See below: A scrotal ultrasound will confirm a diagnosis of hydrocele. While this can be corrected with a relatively simple outpatient operation, if it is not really hurting or bothering you in any way, I would advise against it. All surgery, no matter how "minor", can have complications, and in your case, this is the only testicle you have left, and is the only thing making testosterone and sperm!

Your decision: Probably had a scrotal ultrasound to evaluate testicular or scrotal pain. Pain is unlikely due either to a small varicocele or hydrocele, both are usually painless. Large varicoceles, clearly visible when erect may cause a feeling of heaviness or dragging occasionally low grade pain. Hydroceles have to be large to cause feeling of discomfort from thigh rubbing against it. Avoid surgery for pain.

What are the tests for hydrocele?

Scrotal ultrasound: Is essentially the only reliable test for hydrocele. Transillumination can be used, but is not 100% reliable because subject with a hydrocele may also have a testicular tumor which is not necessarily seen on transillumination.

How do I know if I have a hydrocele?

Ultrasound: You will need to see your doctor to be examined, and then have an ultrasound to verify it. If you have a change in your testicle, a lump or pain, swelling or redness, definitely you should see your doctor.

Please suggest me how to reduce the size of hydrocele by any means?

Here are some. ..: For most men with hydrocele, its cause is still idiopathic and there has been no known means proven to slow down its growth. And the mainstay of its care is still surgery - hydrocelectomy. For other causes such as infections or tumors, their care should be directed to their underlying causes with appropriate antibiotics and timely surgery. So, there is no easy means to reduce its size.

Possibly: Hydroceles can grow over time if left untreated. They may grow very slowly or more rapidly depending on the activity level of the patient.

What can help in the treatment of a hydrocele?

Observation v repair: Many hydroceles are small and asymptomatic and do not merit any treatment. If the hydrocele comes and goes, it is a "communicating hydrocele" which merits repair. If not, but if it is very large, or symptomatic, then surgical repair is usually performed, although needle drainage with or without sclerosis are used occasionally.

Not really: There are many treatments for hydroceles but unfortunately the one with the highest success rate is surgery.

No: No, there is no specific medication or medications that can treat hydroceles. This is generally a surgical problem which needs a needle drainage in the office or surgery to remove the bag of around surrounding the testicle.

Ibuprofen but: It depends on the cause of the hydrocele. If it is acute, then surgery may be indicated. Chronic hydroceles in adults form because of inflammation of the lining of the hydrocele sac. Therefore anti-inflammatory medication such as Ibuprofen may be helpful.

Hydroceles: A rare cause of hydrocele in some developing countries is clogging of the scrotal lymphatic system caused by repeated or chronic infection with insect borne parasites. (eg wuchereria bancrofti). Generally these patients have a condition known as elephantiasis. This differs considerably from the typical "hydrocele".
