Sounds like some sort of dermatitis. Most genital rashes aren't STDs and no STD, including herpes, ever causes this sort of problem. Wrinkled skin by itself is not abnormal.
"Non-STI" HSV probably means HSV1, the usual cause of oral herpes. Half of all adults have positive tests. This has nothing to do with your symptoms. See a doctor or clinic, but don't be worried. Probably not serious. ...Read more

Sounds viral: Sounds like a herpetic type of infection with location and duration of symptoms thusfar. Not much you can do with it being almost a month into it but ask your doctor for some conservative suggestions such as keeping the lesions nice and clean, taking an anti-inflammatory, and perhaps trying something topical with a little anesthetic and Gabapentin or similar. ...Read more

See a dermatologist: The foreskin is not a common place for skin tags. Two possibilities are molluscum and warts. Possible: If you are taking the Valtrex (valacyclovir) to prevent out breaks it is possible to have one while on the medicine. It decreases the risk but doesn't always prevent an outbreak. Probably not herpes: Herpes doesn't cause this sort of problem, especially when it lasts several months. This sounds like garden variety fungal (yeast) or nonspecific balanitis, very common in uncircumcised men. Probably not caused by hand sanitizer, even if that was just before the problem started.
