Differences: Eczema is a chronic inflammatory skin condition due to genetic defects in the proteins and lipids supporting the skin layer/barrier called the epidermis. Ringworm is a skin infection due to a fungus. Psoriasis changes the life cycle of skin cells causing cells to build up rapidly on the surface of the skin. Extra skin cells form thick, silvery scales and itchy, dry, red patches, that are painful.

How to differentiate between eczema and ringworm? Is there any manual protocol to determine whether the said infection is either of two?

Eczema?: Nummular eczema can mimic the look of ringworm. Are you sure that it is eczema and not ringworm though? You can use the traditional eczema treatment on it, if u r sure. Dove sensitive skin soap to wash, aquaphor to moisturize and 1% hydrocortisone directly to that spot. Hope this helps. If you are unsure, seek the diagnosis from your pediatrician or dermatologist.

Not sure exactly: What you're asking. From your description, it sounds like the proper diagnosis, but you didn't mention what you were treated with. If you just started on your treatment, it should start improving in a couple of days; if not, please go back to the doctor.

Itching: Eczema is common caused by contact allergens (gloves, wetness, fur) and causes inflammation which is itchy. The areas you are referring to might have become lichenified or inflamed to the point where they are red, thickened, and painful. This does not discount the fact that skin breakdown from eczema can increase your chance of opportunistic infections like the fungus that causes ringworm.

Fading dark spots: If the fungal infection and eczema have resolved and only dark spots are left, the meds you mentioned are not useful in fading dark spots. Lightening of the skin you see with corticosteroid creams is actually a side effect and goes together with atrophy (thinning of the skin), acne, stretch marks. And they don't really fade the dark spots, just damage the skin. A hydroquinone cream can be tried.
Possibly: The distribution pattern for eczema is fairly predictable, while "ring worm" which is a superficial fungus infection can occur anywhere on the body including scalp, beard, trunk, groin, foot. Both rashes can be itchy, but eczema is really itchy. Eczema responds well to cortisone creams/ointments, while cortisone can potentially worsen ringworm. The ring worm on my hand seems to not go away. But if the ring worm is gone could it be eczema that took its place. It itches like crazy. & with heat?

Likely not ringworm: Skin rashes are common in infants & a single patch of rash can be a lot of things. From drool rash to ringworm to allergic reaction, etc. If it improves with hydrocortizone it is probably not ringworm. It doesn't sound worrisome at this point so you can bring it up with your pcp at the next scheduled visit.
