A note perhaps?: You might be able to write your doctor a note that says what you'd like to say. You can point out that the reason for the note is just an example of how difficult it is for you. But also, it might be a good idea to search for a sensitive psychotherapist to address this problem since my experience leads me to question medication as a 1st strategy.
