My toe has a thick scab in between the small and 2nd to last toe. Used to be athletes feet years ago. Now when I wear certain shoes it hurts severely?

On occasion the little toe has only two. When this happens, pressure from the toes bones on opposing toes causes a callus. This can be very painful, aggravated by shoes and verified by x-ray. I have accidentally created a half-inch-long, dark purple scab on my foot by picking at it. It does not seem too severe. Should I let it be as it is? I don't want to resort to surgery.
Scar: Make sure the scar edges are healing nicely, and there is no swelling or redness developing. I would place neosporin on the wound twice a day, for about a week. This will prevent infection.
