Two possibilities: Either irritation from the pants rubbing on his skin or "jock itch" which is a fungal infection caused by the fungus, tinea cruris. It's not serious and is readily treatable with a combination cream which contains a steroid and an antifungal medication; one such med is Lotrisone (clotrimazole and betamethasone) cream (it's available in its generic also). A problem: I am not aware of any exercise that can result in removal of localized fat deposits. The best reason for liposuction is "a localized fat deposit that doesn't respond to diet and exercise." you may want to investigate this with a surgeon certified by the american board of plastic surgery if the belt fat is bothersome enough to you.
