Verify UTI diagnosis: Most important thing is to verify that you have been truly having recurrent utis as there are other urinary conditions that can mimic the same symptoms. Though a urinalysis can be suggestive, often misleading. Urine culture required to verify. If DX of recurrent UTI is correct, recommend urological evaluation to determine cause. Daily low-dose antibiotic for 6-12 weeks a common treatment option.

Probably not a UTI: There are many causes of urinary pain and frequent urination. A common cause is bacterial infection (UTI) but when antibiotics do not improve symptoms, you either have an antibiotic-resistant infection (would be shown on urine culture) or the cause is simply not a bacteria. It's too involved to describe here, but try looking up "Bladder pain syndrome, " to see if it describes what you are feeling

Be: Be confident that your doctor is doing the right thing. The urine culture that was done shows which antibiotics will work and which will not against the bacteria that is causing the infection. If you were put on Nitrofurantoin again then I am sure the culture showed that the bacteria was sensitive to it. Recurrent uti's are very common in pregnancy, and the fact that you took the antibiotic before does not mean it will not work this time. If you need more reassurance or explanation don't hesitate to contact your doctor. Take care.

Pediatric UTI: This sounds like a job for a pediatric urologist. There are many possibilities, and you have not provided sufficient info for a diagnosis to be made. A 4 yr old needs to be fully evaluated, not only for possible causes, including anatomic defects, but also for the possibility of abuse. See a good pediatrician and get a recommendation.

See below: The most important course of action is make sure the diagnosis of UTI is correct by obtaining both a urinalysis and urine culture (C/S). The latter will verify if a UTI is still present, identify the specific bacteria, and identify which antibiotics it is sensitive to. There are other conditions which can mimic a UTI. If recurrent UTI, further testing and long-term antibiotics may be necessary.

Abx in UTI: Your doctor can tell you what the sensitivities were to the specific bug you were growing in the urine. It is usual for urinary tract infections to respond quickly to antibiotics. Please call your PMD to follow up.

Within a day: Great question. Since uti's are painful, effective therapy can alleviate this symptom within a day. However, other symptoms, depending on the severity of infection, may take up to a week (fever, malaise) or so to abate (eg.Flank pain).If symptoms smolder, inform your prescriber immedately- antibiotics may need to be adjusted. Hopefully, a urine culture was intially done to guide antibiotic choice.

Further evaluation: Can be a complicated issue. You need to discuss the reasons with a urologist. The bacteria involved may be difficult to treat, or there are structural urinary tract problems. Sometimes longer courses of suppressive antibiotics are helpful.

Many antibiotics.: Optimal 1st antibiotic choice for UTIs are trimethoprim-sulfa (Bactrim), Cipro (ciprofloxacin) & Macrodantin. Amoxicillin, Augmentin & sulfonamides + sometimes tetracyclines (like Docycycline) used 2B first choice but more & more UTIs are resistant to them hence the change. Also reason why we recommend urine cultures co-incident with urinalysis to insure bacteria causing UTI are sensitive to prescribed medication

Probably not: Sometimes an antimicrobial will work even if stardard sensitivities say resistant because of higher levels in the urine. But doesn't sound like it here. Do you know what the bug is and what it is sensitive to?
