Common things...: Many things can cause this including anal fissures, an allergy, or something more complicated. If your baby has a bloody bowel movement, this is definitely a reason to make an appointment with the pediatrician. Bring in the bloody diaper as well. Perhaps: If they baby is eating well then it may not be a problem. On the other hand a fussy constipated infant may be sensitive to the formula being used. You may try a formula made for sensitive infants. You can also try giving 1 oz nursery water twice a day. Do not give more water than that. Not useful: It's OK to give some water, but it probably is not going to be helpful. A 3 week old may go several days without a BM and still has a normal BM and has no problem with it. If he does get fussy and seem to have problem, pushing out hard BM's, then contact his doctor for recommendations. Pediatricians deal with this a lot, so follow his/her advice. ...Read more

No: Never give a baby water at this age! Water intoxication can happen in even small amounts. Think of it this way, formula and breast milk have just the right amount of sodium ( or "salt") in them.
