How sure is the dx?: Reactivated mononucleosis is unusual. Most cases of mono are caused by EBV but CMV also can cause. While exceptions exist, usually mononucleosis confers lifelong immunity. Ask for diagnosis: Palpable neck nodes that don't go away and large enough to be consider pathologic should be a concern. They can stay that way without disease for some time but certain diseases can do that, unfortunately like some opportunistic infections, mycobacteria but could be a manifestation of other inflammation including aids and malignancy. Sounds concerning: Are all the nodes on the same side? Are they growing? Do you have hoarseness? Ear pain? Difficulty swallowing? If so, you should have a laryngoscopy. Have you have weight loss, unexplained fevers, night sweats? It is very unusual that you can feel nodes that are not seen on ct. Maybe have a second doc look at the ct? ...Read more

Yes, not abnormal: Once inflamed and enlarged, sometimes lymph nodes never return to previous normal size. If you're otherwise well, it doesn't mean anything if you can still feel them. Not likely: Lymph nodes in the neck enlarge for lots of reasons. Often it's infection, sometimes immunizations. If they're small (< 1/2") they may just be scarred from a previous infection.
