Orchitis: Are you on antibiotics? I would follow up with a urologist usually after 1 week of antibiotics the organ/area with an infection should be back to baseline so if you have persisting issues at this point you need further evaluation. ...Read more

See answer : The usually course of epididymo-orchitis, once treatment has begun, is that the pain and discomfort usually resolve or markedly improve within the first 2-3 weeks of antibiotics but reduction in swelling takes much longer usually in the range of 6-12 weeks and sometimes never quite returns to its pre-infection size. ...Read more

No harm: Masturbation probably won't hurt anything. At your age (19) by far the most common causes of epididymitis are chlamydia and gonorrhea, although these aren't possible if not at risk for STDs (new sex partner etc). Cephalexin won't cure chlamydia and is borderline for gonorrhea, but might work on other bacterial that can cause this problem. Re-check with your doctor if any doubt about all this. ...Read more

May be...: Effective response to antibiotics suggests you got std-related or uti-related epididymo-orchitis. For young men, it would more likely be std-related; for older, uti-related. But antibiotics would not interfere ejaculation / orgasm.
