Here are some. ..: I answered this Q not long age. But I repeat. Nitrofurantoin is still a good antibiotics for uncomplicated acute UTI or chronic suppressing antibiotics for recurrent UTI after evaluation despite having been used for decades. So, use antibiotics with better tissue penetration such as flouroquinolone, cephalosporins, tetracyclines, etc. But using antibiotics for 30 days for orchitis is uncommon... ...Read more

Again: Nitrofurantoin, as I commented on another doctor's answer to your question, is not a first choice for orchitis because of very poor tissue penetration. Not much difference: Slight nod to levofloxacin, but they cover pretty much the same bacteria. Most non-STD epididymitis at your age is not bacterial, so antibiotics may not help. How certain is the diagnosis? If you just have testicular pain without obvious enlargement or tenderness, I would be suspicious you don't have true epididymitis. Maybe chronic pelvic pain syndrome? See urologist or ID: Presumably tested for STDs? Gonorrhea and chlamydia are the most common causes of epididymitis or orchitis at your age. If diagnosis of non-STD bacterial infection is correct, treatment depends on which antibiotics are active against that bacteria. Usually Bactrim (sulfamethoxazole and trimethoprim) or a quinolone would work, but not always. Check for TB, testicular cancer.
