Stretch injury: If your piriformis pain is secondary to pelvic imbalance and it's constantly stretched, the last thing you want to do is stretch your piriformis. It will aggravate even if it feels good stretching it at the time. Find a physical therapist who can do myofascial release or facilitated positional release and teach you relaxing exercises.

Exam: The primary diagnostic tool is a detailed exam. A nerve conduction test specifically looking for piriformis syndrome can confirm. An injection in the piriformis area is used and can be both therapeutic and diagnostic. Physical therapy can be helpful in many cases.

Get Another Opinion: First, get a second opinion. Schwannomas are usually amenable to surgical correction, unless in your case there is an unusual circumstance. Depends on cause: This is often a lumbar radiculopathy with compression of the nerve as it exits the spine. It often improves on its own with medication and physical therapy. However, surgical decompression is sometimes required.

Get an exam: If there are neurological findings it can change what might be reasonably tried. Chiropractors, acupuncture, physical therapy, gabapentin, trigger point work, and believe it or not emotional work can all help as well as stretches. I like the ones that stretch the iliopsoas muscles.

Conservative care: Unless the disc is so large on MRI scan that nothing short of surgery will help, patients should be started on non steroidal anti-inflammatory meds, muscle relaxants, pain medications, steroids if necessary. Try pt for some time. If symptoms persist, then I agree that epidural injections and surgery should be considered.
