Obstructed?: Obstruction of the upper part of the ureter or within the kidney could do this. E.g. from a kidney stone or papillary necrosis. That is, the infected area is not draining, and so no bacteria or white blood cells in the urine. If so, it's quite serious. Partially: Very possible that a low grade infection caused the initial symptoms, then became full blown pyelonephritis, which requires initial IV antibiotics and long term treatment (at lest 3-4 weeks). See below: Hopefully you had a urine culture done and are on an antibiotic to which the causative bacteria is susceptible. Treatment should last 10-14 days depending on the antibiotic chosen. This is serious: A nephrologist or pathologist needs to look at the urine to confirm the presence of red cell casts. If they are indeed present, you have glomerulonephritis of some sort. Since you state that you have systemic lupus, this is the obvious first consideration, and the presumptive (and likely) diagnosis is lupus glomerulonephritis. Yes and yes: Pyridium (phenazopyridine) dyes the urine orange & that color can interfere with the dipstick test for leukocytes. But the gold-standards for diagnosing UTI's is looking for leukocytes under the microscope (for immediate dx) & culture. Pyridium (phenazopyridine) does not interfere with those. Pyridium (phenazopyridine) only reduces symptoms & doesn't treat the infection. You might have pyelonephritis, which can be serious. Seek medical attention ASAP! ...Read more

I believe so: I am not a radiologist, but as a pathologist I can assure you that I've seen cases of pyelonephritis that i'd think are are too subtle anatomically to show on x-rays. White cell casts in the urine establish the diagnosis, and if there's white cells in the urine and flank pain and fever, the presumptive diagnosis is kidney infection, imaging or no. ...Read more
