Probably: Best thing is to reach out and call your doctor. If you go to an er who does your urine test, blood test and temperature, they can also call your doctor after that. If you still have a white count, fever, and dirty urine, they will admit you. ...Read more

Acute infection: If you cannot find a doctor who will see you then it would be prudent to go to the er. You don't say why you are taking Metronidazole (flagyl), but that is not likely to be effective if you actually have an upper uti. Hope you are better quickly. ...Read more

Obstructed?: Obstruction of the upper part of the ureter or within the kidney could do this. e.g. from a kidney stone or papillary necrosis. That is, the infected area is not draining, and so no bacteria or white blood cells in the urine. If so, it's quite serious. Or maybe infection with a bacteria not detected on routine urine culture, but that's very rare. Keep working closely with your doctors. ...Read more

Partially: Very possible that a low grade infection caused the initial symptoms, then became full blown pyelonephritis , which requires initial IV antibiotics and long term treatment (at lest 3-4 weeks). See below: Hopefully you had a urine culture done and are on an antibiotic to which the causative bacteria is susceptible. Treatment should last 10-14 days depending on the antibiotic chosen. It is a good idea to have a renal ultrasound done to be sure you do not have any kidney stones or obstruction.
