Obstructed?: Obstruction of the upper part of the ureter or within the kidney could do this. e.g. from a kidney stone or papillary necrosis. That is, the infected area is not draining, and so no bacteria or white blood cells in the urine. If so, it's quite serious. Partially: Very possible that a low grade infection caused the initial symptoms, then became full blown pyelonephritis , which requires initial IV antibiotics and long term treatment (at lest 3-4 weeks). See below: Hopefully you had a urine culture done and are on an antibiotic to which the causative bacteria is susceptible. Treatment should last 10-14 days depending on the antibiotic chosen. It is a good idea to have a renal ultrasound done to be sure you do not have any kidney stones or obstruction.
