Hard to tell: The temperature of 38c (100.4 f) is not really concerning, but i would pay attention to how he feels. If he seems to be less energetic, not eating well, or having trouble sleeping, he should be checked by the doctor. Likely normal: Many normal children that age have quite variable appetites i would suggest that you keep a daily listing of all that she eats and drinks. After 1-2 weeks, talk to your doctor's nurse to see if they have any concern and whether they want you to bring her in to be seen. ...Read more

Yes: High fever and lethargy can be symptoms of a virus, or a serious bacteria infection. When she is treated for an infection, the antibiotic should be working within 3-4 days.
