He faints with a simple needle stick. Without adrenal hormones, stress can cause circulatory collapse. With him, a tattoo is dangerous. Of course with most people just getting stuck with a needle is a minor stress, tolerated well and little risk.

Consider fungus: Without actually laying eyes on the rash and examining it properly, it's nearly impossible to tell for sure. But one thing that pops into my head is the possibility of a fungal infection.
