Cardiac arrest: If you are warm and not breathing and no chest compressions, 3 minutes approximately will yield irreversible brain damage, if you are hypothermic it could be signficantly longer.
Possibly: Any time your heart stops pumping, the blood in your arteries and veins stops flowing. Therefore all of the organs are not getting the blood they need to function. The brain is a very sensitive organ and can get damaged if it is not receiving proper blood flow. There are ways to check the brain for such damage. Probably not, but...: That is dangerously high bp! If it remained like that for more than 10-15 minutes, given those symptoms a trip to the er is in order!

Yes except heart: B12 deficiency can cause a variety of neurological symptoms including "derealization"; it can adversely affect brain function but that damage should be reversible. Not sure: Adults who are deprived of oxygen due to cardiac arrest for over 5 minutes usually suffer brain damage. However since she did not have a cardiac arrest and, despite very low blood pressure, she may have had enough cerebral circulation to avoid significant brain damage. Sounds like it: The missing usual symptom is the sensation that you are going to die (during attack / as result of it). Unfortunately, only long-term resolution is to address whatever is triggering them. This requires seeing a therapist/psychologist to identify problem. Your dr or a psychiatrist can work with you & therapist to rx meds to reduce severity of symptoms while working on problem. Get seen.
