See below: With your description, this is likely viral in nature. Abx are way overused in our society and according to the infectious disease society of america, 98% of all sinusitis is viral and does not need abx. Ear infx are also often viral and the pain you are feeling is due to the fluid behind the ear drum. Also, yellow or green mucous is non-specific and does not always mean bacterial. Chronic Cough: 65 year old East Asian American gentleman presents with persistent cough he believes started with pharyngitis and developed into severe coughing. Coughing lasted about 2 weeks but now has persisted at night. No complaint of fever, night sweats, hemoptysis. Cough relieved by sitting up. Assessment and plan. Chronic cough that does not go away can be a sign of tuberculosis. You need to have a TB test placed immediately and examine sick contacts, etc. Studies show that the most common causes of chronic cough are upper airway syndrome-postnasal drip (trial of antihistamines indicated), GERD (empiric treatment of GERD indicated) and asthma (trial of bronchodilators indicated). Studies show category C evidence that physicians should order a chest x-ray for non-smokers with a chronic cough (>8 weeks) who are not taking an ACE inhibitor.
