Several : Several things can be occuring 1. A trigger thumb where the tendon gets transiently locked causing the distal joint ( near the nail) to become stuck. 2. A swollen joint from being contused ( in this cases it sounds like the mp joint) causing the joint to glide less fluidly 3. An injured collateral ligament at the ulnar aspect of the mp joint 4. Up or down bend.: It is quite possible that you injured the toe and were unaware of the injury. This could be a low grade injury to any one or several areas of the proximal phalynx of the third toe. Included, but not limited to are the tendon, bone and capsule. Discoloration and swelling are signs of a more severe problem. If it is a pressure point where you normally grip the pencil or pen, it is probably just a callus. Rest on that area or loosening your grip on the pencil will help it disappear. Stop pulling...: Just a little more clarity is needed. When you pull down on it, where exactly does it hurt? Exactly what are you pulling down on? It : It sounds like you're supinating, which is typically associated with a high arch foot structure. Folks with this type of foot don't make complete footprints in water when they get out of the shower. You only really see the sides of the feet. Such a foot structure is fairly rigid, and lacks the shock-absorbing component of a more flexible arch. Your body tries to compensate for this structural imbalance by overusing muscles to try to stabilize your feet during the gait cycle, which leads to muscle fatigue. A podiatrist would be able to give you a biomechanical examination of your lower extremities and diagnose which muscles are being overworked. In addition to stretching exercises and some shoe recommendations, most patients with such an imbalance do well with a pair of biomechanical orthotic devices which are worn inside the shoes and taken from shoe-to-shoe.
