Why don't people with hypopituitarism require mineralocorticoids?

Different control: Hi. Good question. As you know, cortisol production by the adrenal cortex is exquisitely regulated by ACTH from the pituitary. In ACTH-deficiency (hypo pit.), cortisol must be replaced. The primary regulator of aldosterone production is the renin-angiotensin system, which does NOT involved the pituitary, and functions just fine in hypopituitarism. ACTH will acutely stimulate aldo, but is not req'd ...Read more

Could you tell if a person has hypopituitarism just by looking at them?

No: Hi. No, you can't tell hypopituitarism just by looking at the patient. There are a couple of pituitary hormone excesses you can tell just by looking at & examining a patient (Cushing's and acromegaly stand out, and you can get a pretty good clue for a prolactinoma). But that's not the case for hypopituitarism. You need an index of suspicion and to do the appropriate tests. ...Read more

Would someone with hypopituitarism be able to make adrenaline or noradrenaline? Wondering for sports.

Absolutely, YES: Hi. People with hypopituitarism have no impairment in adrenal medullary function. Caveat: if your pit. tumor was Cushing's, and ACTH production couldn't be cured, and you went to bilateral adrenalectomy, then of course you'd have no adrenal medullary function. That's the only caveat unless you somehow had hypothalamic damage. You're probably FINE with sports. Go play! ...Read more
