The approach...


Both are forms of "behavoir modification"; and if you and your physician feel this is going to help you achieve your weight loss goal than go for it. I believe in a more traditional approach(diet modification(cut out white flour/sugars), 3-4 times/week of moderate activity(like walking; or other low impact activity) and just drink water ([8]-8 ounce glasses/day). Operant conditioning is a form of behavior modification. Behavior modification is based in part on these basic principles: If someone receives what is considered a reward after engaging in certain behavior, that behavior will be strengthened or occur more often. If someone receives no reward, that behavior will weaken or occur less often. It is up to you to set up what behaviors get what rewards.

each case is different, but in my practice I saw a lot of cases when people simply do not have or do not know other ways to feel good. I also believe that physiology plays a big role: possibly, they need stronger stimulation to get the same effects from dopamine. I have several approaches to treatment; including "physiological extinction" or manipulating with dopamine level

starts by using a drug that briefly gives you pleasure or relieves distress. The most addicting drugs produce tolerance -- more drug is needed to get the same effect -- & cause withdrawal reactions -- you feel terrible as the drug leaves your body. Addictions are misguided attempts to feel good. They are temporary & can cause more problems than they solve, including permanent brain damage.

No simple answer


Sometimes people inflict physical pain on themselves to avoid feeling emotional pain. Sometimes pain has been associated with sex & therefore becomes associated with pleasurable sensations. Sometimes people inflict pain on themselves out of guilt and the punishment feels good (or at least better than the guilt). It is possible that pain releases dopamine, which often mediates other addictions.

Addictive behavior has common underpinnings despite the various chemicals, physical or mechanical, or ritualistic, behaviors may initiate them. Areas of the brain are involved similarly regardless of the addicting substance. Dopamine is a prime mediator of addiction.

Substitution.


Progressive substitution of safer habits can be helpful. For instance, substitute nicotine gum or lozenges for cigarettes. Then slowly substitute spearmint gum or breath mints for the nicotine gum or lozenges. Substitute carrot sticks and celery for chips and candy. Substitute playing sports or other athletic endeavors such as walking for betting on sporting events.

Not sure what you mean by strange. I guess all addictions are strange in their own way. Addiction is strongly genetically influenced. As with anything genetic, each individual may or may not get the trait, depending on relative contributions from each parent and their side of the family tree.
