No need: Azithromycin does this from time to time, usually when a single large dose is used for some infections, and the effect of early vomiting has been studied. That research shows that as long as the drug is retained at least 30 minutes before vomiting, enough is absorbed and retained to do the job. Ideally you should..: wait 2 hrs before ingesting food. This is because food may reduce the bioavailability (availability of the antibiotic in the body) and therefore make it less effective. If around 2 hrs have passed it's okay to have food. If you ate a little sooner than 2 hrs, don't worry - it probably won't make a HUGE difference (so don't take an extra pill!). It depends: If you threw up the medication, it may not have had time to be digested and systemically absorbed to be actively treating your infection. Get re-tested to be safe and you may need re-treating, assure your sexual partner has been treated too, or you will get re-infected. ...Read more

Not recommended: Antibiotics and combined Alcohol use are never recommended as the Z pak (azithromycin) is likely not going to be as effective for chlamydia use. My rationale is that Z pak (azithromycin) is metabolized in the liver by Cytochrome p450 and Alcohol increases this same enzyme that degrades a Z pak (azithromycin). Probably: Food in the stomach usually site there for 1-2 hours prior to entry into the rest of the GI tract. This means there is a good likelihood that you expelled the azithromicin. There is no harm in having a little extra in your system so given what you describe and that you can now eat, take an extra pill. ...Read more
