Infection?: Red hot swollen spots make me think of infection, perhaps abscess or folliculitis, or sebaceous cysts. Abscesses don't usually go away by themselves but rather need to be drained. Folliculitis might come & go but not usually 4" in diameter (that's huge!). Not common to have several sebaceous cysts in one area. Go see your family doc for evaluation, testing & treatment. Depends: If by "hot spot" you mean an area of redness, swelling, pain and heat this could be serious and you need to see a nurse or physician. Preferably while you are having the feelings! So a walk in clinic if you don't have a doctor who will do same day appts. If it's just a feeling of warmth, then it probably isn't much of anything.
