Need Evaluation First: Post Menopausal Bleeding (PMB) or any abnormal bleeding >35 years old needs to be addressed. If your bleeding is heavy enough to cause anemia (light-headed, dizzy, weak, tired) you should call your Doc or head to the nearest ER. If not, your Doc will need a pelvic US and labs.. Call and see if you can have these done before your appt. An endometrial bx is needed to rule out abnl tissue, good luck

Irregular periods: Having periods that are rare with sometimes several months between them may be a sign of polycystic ovarian syndrome aka (pcos). On the other hand your current 3 months of bleeding require a proper work up. You should a complete blood count to check for anemia and an ultrasound to check for anatomical abnormalities. In short, it is time to see an obgyn.

How do I stop excessive menstrual bleeding? Clots and more clots

Multifacted: Excess bleeding could be a hormonal imbalance or it could be a fibroid or polyp in the uterus or cervix. A visit with your ob/gyn will help possibly narrow your diagnosis. Sometimes excess bleeding can even be precancerous cells in the uterus. I would recommend you see a physician and be evaluated.
