This is serious: You have hemochromatosis until proved otherwise, and this is probably why your alt is disturbingly high. Just how much of a workup you want is between you and your physician; a gene study, imaging, and/or a biopsy may be in your future. Not necessarily: Serum iron levels are not a good test to make treatment decisions as it is very variable and do not reflect inappropriate iron deposition. Ferritin level is the standard. If there is a concern for hemachromatosis genetic test is available. If indicated, liver biopsy with iron studies can be considered. ...Read more

Iron: Not sure what your reference values for labs are but the ferritin does not look low. See if you can get an iron percent saturation that will be useful. If it is above 55%, you should look for iron storage diseases like hemochromatosis. ...Read more

Big picture: High iron / iron sat is meaningless as a measure of storage iron if you're taking ferrous gluconate. Why were you on it in the first place? If it's to correct heavy monthly iron loss, ask your physician whether to continue it prophylactically so you don't get a recurrence. ...Read more

Agreed: Based on a low Ferritin and Iron I too would expect a high TIBC. However, tests are not always in agreement with logic. On face value it would appear you are iron deficient, however I do not know you blood counts and several other labs. Fluctuations: Serum iron is sensitive to diet, supplements, injury and a host of day-to-day variables. Low serum ferritin is usually a more reliable indicator of available stores, and if mine were quite low, i'd be looking for why i might be losing iron / bleeding rather than have the serum iron think i'd been misled. ...Read more

Iron panel- UIBC: 109 (low), Iron serum: 170 (high), Iron saturation: 61% (high), TIBC: normal, Ferritin: 40 (normal). What can cause this?

May be normal: All laboratory results need to be interpreted in the clinical context and the doctor who ordered the tests is usually in the best position to do that. Used to chronic blood loss. I fixed that. Ferritin was low, took iron pills. Now ferritin normal. But serum iron is high. Stopped pills. Implication?

Manage it: I'm really glad you found the cause of your chronic bleeding. The serum iron isn't anything to worry about while you're taking oral iron. Each pill causes a spike in the value. Keep following this, and I"m glad you're better. Best wishes. ...Read more
