Needs to checked out: In addition to your heart I might want to assure your liver and lungs were fine.   Irritation of the diaphragm can cause hiccups and be associated with referred pain to the shoulder.   I had a patient once with an amoebic liver abscess that presented with shoulder pain.   Time to see your doctor!  Especially given the intensity of pain reported.

Possible: Try making a chicken wing like motion and lifting the shoulder up. If you can not lift it the first 20 degrees, you may have torn the suprapsinatus tendon which is commonly injured. However, you might just have a tendonitiis if you don't remember a trauma. If you can lift it 20 degrees but not further, you may have injured another rotator cuff muscle. See a family dr or sports dr for further eval.

Muscle spasm.: It could be muscle spasms of the small muscle between your shoulder and rib cage. Try a comprehensive stretching program  of your shoulder and trunk.

Bursitis: If there is no history of recent sprain and motion of the shoulder affects the discomfort, bursitis is likely. Try cold or hot applications, liniment, rest and , if you can take them, nsaids like Ibuprofen with food.  If symptoms persist or worsen, an evaluation by an orthopedic surgeon would be best.

Varies: There are a number if possibilities, the two mist common are a cervical radiculopathy or a rotator cuff tear.  A cervical radiculopathy is an irritation if a nerve in the neck causing pain running into the shoulder.  A torn rotator cuff does not always need a injury to cause, can be degenerative.  I would not hesitate to be seen.  An exam can usually determine the likely cause.

Diaphragm spasm: The diaphragm and the deltoid muscles share a common ennervation. When you have diaphragm spasm with coughing or sneezing the pain is referred to the shoulder, usually right sided.
