Possible vasculitis: I would have an ANCA panel checked, especially with you saying you're having hematuria and proteinuria. There are a few conditions we refer to as pulmonary renal syndromes and they are all a type of vasculitis. Some of the names are Wegener's and Goodpasture's.

Concerning nodule: Solid nodule in association with cyst is concerning. Needs follow up with either CT or MR with and without contrast to characterize. Especially if you have no prior imaging to compare to.

Regroup: Regroup and review the data. A careful review of the upper urinary tract imaging and bladder biopsy/cytology/fish studies should be reviewed. You may need to be re-studied, have new cultures done, etc in order to find your diagnosis.

Seems like UTI+-: Flank pain in female comonly asociated with kidney infection. All other symptoms are suggestive of urinary infection (UTI) that dose not involve the kidneys and in general is a benign condition unless is too frequent.

? UTI: Acute cystitis should be considered. Drink plenty of fluids until you can get a urine analysis and a prescription for antibiotic if infected. Meanwhile get over the counter AZO which will help the painful urination a bit (but will color your urine red). No need for ER (too costly for cystitis)but go to your doctor or to walkin clinic. Not necessarily: Did you get a CAT scan of the kidneys and ureters already? I realize this may be causing some anxiety for you, but we don't want to jump to conclusion too soon without having all the test results.

See a Doc: If dipstick & labs (do you mean microscopic urine analysis) are normal then what do you mean by terminal hematuria? . Combination of diarrhea with gross or microscopic hematuria especially with some other symptoms such as skin rash, arthritis or blood in the stool and history of travel needs evaluation & management. My suggestion you should talk to your GP.

Need to see urolo: Gist. Your symptoms are in many ways consistent with overactive bladder syndrome. It's possible that you always have microscopic hematuria. See nephrologist: Would discuss with your rheumatologist and nephrologist. Most ofter with lupus blood in the urine is due to lupus kidney disease called glomerulonephritist which is treated with your rheumatologist and nephrologist. There are other causes of hematuria including infection, kidney tumors or kidney stones. I assume all of this has been evalauted

See urologist: Cancer of the bladder while unlikely at 26 is increased in smokers and gross hematuria is one of the symptoms. You need to be seen by a urologist ASAP and remember there is no better time to stop smoking than now. Best wishes.

Surgery: With a presumed endometrioma (a fluid filled "cyst" formed from new or old endometriosis) needs removal because of its size and because it is causing the symptoms you stated.

Complex: Depends on age and gender and medical history and if blood is visible. Seeing a urologist for older folks is the place to start. Does the person smoke? The urologist may ask for a CT scan and if their workup is negative, may need to see a kidney doc. Sometimes this is a frustrating condition, but most often cause can be found. Talk to your MD and get to see a urologist.
