Infection: Our most common concern is infection. It is a tumor necrosis factor inhibitor which is a very effective anti-inflammatory biologic, but we always use caution because of concern of side effects such as infection, neurologic problems, and also sometime malignant problems.

Equal to other a-TNF: Overall, all a-tnf drugs are equal, but for some patients, one may work better than another, and for some conditions, such as uveitis, Remicade (infliximab) may work better, Remicade (infliximab) helps about 75% of patients, and has the advantage of being able to vary the dose, if a higher dose or more frequent dose is needed. Higher doses do have a higher infection risk.

See details: There are several difference but the only real one of importance is simponi (golimumab) is given subq and Remicade is given iv. They are both very effective.

It has side effects: All medicines have side effects. You have to decide if the benefit you will get from it is worth the risk. The risks are well detailed in the package insert. We can't sugarcoat it but we carefully monitor and we try to avoid the side effects. I often recommend a short three month to six-month course to see if the benefit you get is worth the risk.

Not much: You just sit in an infusion chair for 2 hours. It is really an uneventful infusion. With subsequent infusions, these is a risk of infusion reactions.

What to know and expect before my first remicade (infliximab) infusion friday?

You will: You will an IV placed in your arm to receive therapy and you may receive some medications before you start like tylenol (acetaminophen). The Remicade will be admistered slowly. If you feel any unusual symptoms such as icthing, chest pain, etc, let your infusion nurse and they will probably slow rate down which helps. Typically you will in for about 3 to 4 hours. Remicade (infliximab): You can go out of town in developed countries (US), but would take a thermometer, for temperature. If you do get ill, diarrhea, pains, fever, cough, during your trip seek attention within 24 hours at an urgent care/ER and always have your doctor's number with you when traveling. Immunosuppressed should best avoid swimming in lakes, oceans (oxymetazoline) (bacteria), dusty/desert areas (mold), sleeping on the ground

Many people have: This drug is used long-term and has been very effective. We are always aware the side effects and we try to minimize them. We have to look at the risk of illness such as rheumatoid arthritis versus the benefit versus the risk of the medication. I will suggest let's try a short 3 to 6 months see how you do and then you decide if it's worth it. This will give you a short-term experience decide if you're willing to take the risks.

Several: There are several alternative biologics include other TNF agents, Orencis, Rituxan and Actemra (tocilizumab). You need to discuss this issue with your own rheumatologist.

Is it necessary to take azathiroprine while taking remicade (infliximab) infusions?? What do the two drugs do to help you? Or help each other.

It depends: Azathioprine can reduce the rish of infusion reactions to Remicade (infliximab) and can inhibit the production of antibodies to Remicade (infliximab) that can compromise its effectiveness. However, it is not absolutely essential to take it while on Remicade (infliximab).
