Might be abscess: This may not be a hemorrhoid. Sounds more like a peri-anal abscess. Please see your doctor as this may need to be surgically drained.

You should go see yo: You need to see your doctor for physical examination. It may be an inflamed hemorrhoid (Piles). This will need treatment otherwise you can get severely constipated and get into further trouble because of pain and inflammation/infection. Local application of medication is necessary to help you heal this area. So you should see your PCP right away in 24 hours or go to the ER for treatment.

Don't: Henmorrhoids rarely get infected. If a swelling near the anus burst and discharges purulence it is more likely a rectal abscess and may have a fistula associated with it, which may require surgery to prevent recurrence. See a colon and rectal surgeon for evaluation.

See a surgeon: See a surgeon and get the hemorrhoid or from that description the abscess or fistula treated. This will involve antibiotics and a surgical procedure to carefully examen the area, "exam under anesthesia, " and treat it at the same time.

See a doctor: If you are concerned about an infection, over the counter products will not be effective. If there is pus or infection, see a doctor for proper diagnosis and treatment.

Over the counter: Creams for treatment of hemorrhoids are several, (e.g. Preparation H) you can use those, but keep the area clean and dry after bowel movements, cut your fingers short, avoid constipation, etc. If despite that you still have symptoms, you better see your doctor, online / self treatment then, won't help

Go to the Doctor: Usually one learns the 'three s' approach to managing their hemorrhoids at home. But, given your symptoms there is absolutely nothing to tell you other than that you must be seen in person, examined, and possibly sent on to surgery. Antibiotics treat infection. They do not treat hemorrhoids.

Gastroenterologist: Whenever one has bloody stool, it is important to see a gastroenterologist to sort out the cause. If you have not had a recent colonoscopy you will probably need one. It is possible that this may be a diverticular bleed. You should not delay in seeking help. If the bleeding is severe, go to the emergency room. Good luck.

Not an STD: If you are a virgin and have not engaged in sexual intercourse of any kind, you do not have a sexually transmitted disease. You could very well have an ingrown hair or a host of other skin lesions. See your doctor.
