Maybe: External hemorrhoids usually hurt and occassionally bleed, internal hemorrhoids usually bleed without pain unless they prolapse to the outside. More info; get check: Any anorectal coitus? Possible irritation or even early herpes infection. Any constipation or hemorrhoid history? That could be a factor. Anal fissures: Would cause bright red blood from the rectum. Darker blood suggests bleeding from further up inside the colon. If he has not had a colonoscopy he needs one.
