Lots: Fluid retention from lymph issue, prior trauma/injury, nerve issue, back pain/injury, hip issue, knee issue... Get hint? In 20s there are a lot of potential causes that are way more common than als. Neurologist: I would recommend seeing a neurologist. The heaviness sensation is vague and hard to sort out sometimes but it can be what's called "functional weakness". That can occur as the result of a central nervous system problem. If your neck hurts then there may be some issue with the spinal cord at that level? Diabetic Amyotrophy?: With a history of diabetes, especially if severe or uncontrolled, a rare but occasional cause of leg weakness is diabetic amyotrophy. This usually involved one or more nerves in the legs. It is different from peripheral neuropathy since you may experience weakness. A more likely cause would be a radiculopathy, which is a pinched nerve around the back. Only way to know is a good exam and EMG test. ...Read more

Back pain: You should be evaluated to determine the cause by seeing an orthopaedic doctor. Many times some simple things can be reccomended to alleviate your symptoms ,provided your spinal structure is intact. What you describe is a bit too complicated to suss-out online but I'd suggest you find a good neurologist and get a better answer.
