Depends: Where are you using the heat pad? If the back, then 20-30 minutes but not at scolding temperatures. If it's the jaw, probably not a good idea if you just had surgery. Days to years: I presume you're suffering from a condition called erythema ab igne. The pigment change can take days to years to resolve depending on your background skin color and other factors. Moisturizers and sun-avoidance may help.
