It is unlikely: In general since heart attacks are based on atherosclerosis of the coronary arteries that takes many years to develop an 18 year old is unlikely to have a heart attack. There are other heart problems that an 18 year old could have, and multiple other causes of chest pain. As always it is impossible to say what a symptom means for you in this kind of forum.
