Hello. : Hello. First off, I cannot tell you why your scrotum is tight and the reason for the dull pain you are experiencing. The cremasteric muscle is situated in the spermatic cord structure. When the cremasteric muscle is relaxed your scrotum will be relaxed. The cremasteric reflex causes the muscle to contract in response to things like the outdoor temperature and being sexually aroused. If you are masturbating excessively, slow down. If you are wearing jeans or briefs that are tight, wear baggier pants and boxers. If you are bike riding a ton, take a break. If it is cold out, make sure you are warm enough. The issue may be quite benign, however if it doesn't resolve or the pain increases, please see either your regular doctor or a urologist. ...Read more

See Dr.: Continue to see the Dr for your work up. The fact that the urine and MRI are normal are good sings, it may be a more topical issue from a dermatitis that could be affecting superficial nerves. The low libido is often psychological and therapy may help you. Best wishes. ...Read more

Questionable treatment, unless indirect inguinal hernia shown first.: Testicular &/or scrotal pain can notoriously be difficult to manage and often does not respond to epididymectomy. Certainly removal of testicle should not have been performed unless testicle was very tender. Various regimes utilized by pain clinics should have been tried first & certainly should be tried now including possible acupuncture. There is also possibility of referred pain. Surgery often does not help with this kind of pain unless it is due to intermittent trusts torsion in which case fixation of testis, to prevent twisting of spermatic cord, takes care of problem.
