I have severe headaches, back of head, lower right side. Sometimes after too much alcohol, sometimes no trigger? The pain can last for days. Help.

Needs investigation: You need to check with your pcp, who should check your medical history, family history and any risk factors. You will possibly need ct or MRI brain to rule out any organic pathology. You also may have some withdrawal headache from the alcohol, or tension headache. It's important to know what alleviates the headache and what makes it worse as well. Hope you feel better.

New headache: Headaches can be a concerning symptom and can have many causes. Some are deserving of emergent treatment. Warning signs are chronic headaches that suddenly change in quality or intensity, multiple headaches a week, new onset headaches that affect one's ability to function, associated vomiting, associated dizziness/double vision/neck ache/fever/weakness, or the need to take daily pain medication.

Neuralgia: Your location may fit the anatomy of the greater occipital nerve, and could be a problem coming from where the neck and base of skull come together. Alternatively, this may be referred pain from neck arthritis. Focal occipital headaches may also be seen in thoracic outlet syndrome. Get a physical exam and then appropriate therapy.

Awakening Headache: Cluster, migraine may produce a headache that is present upon awakening. It is important that the doctor first identify and exclude a secondary headache and determine the proper diagnosis. See headache or orofacial pain specialist for evaluation.

No simple answer: The most common cause of achy pain around the eye that I see, comes from irritation of some nerves in the upper neck near the base of the skull. When they are irritated, there may be a tender spot to pressure over them in the neck but the body may be most aware of the pain felt around and behind the eye - called referred pain. For this particular cause, tratment must be delivered to the neck.

Headache: Difficult to say what this could be. Headaches can be differentiated into those neurological versus musculoskeletal or inflammatory. If you have never had headaches before it is worth seeing a doctor for a more complete examination and history. Keep track of what makes it better, and what makes it worse, and try to use one of the free headache apps so as to keep a good diary of your symptoms.

Occipital neuralgia: H/A in the back of your head about 1.5" off of the midline are called occipital neuralgia. Greater occipital nerve is inflamed and causing the pain typically. Underlying cause, however, is likely to be a problem with upper cervical discs. With "painkillers" not helping, have you have been seen by a headache specialist, rule out other problems. If not evaluated, see MD. Contact me if needed.

Severe Headache: That persists despite medication suggest that you seek medical attention as soon as possible to make certain that something more serious is not going on.

Go to ER.: If this has happened before, then it is likely from migraine.If this is new especially when there has been a history of recent head trauma, then you should consider going to the ER now.

To the ER: I assume if you had MR venography that shows narrowing to full occlusion you have already saught care. This can be related to a cavernous sinus thrombosis which is severe vision threatening condition. Please make sure you follow all the guidelines set out by your physicians. Good luck

See below: Any sinus congestion? Earache may have been part of URI including sinuses, which can cause referred pain to back of head. Try nasal irrigation with saline spray. If not being treated for high blood pressure, might add decongestant. Nsaid (advil/motrin/aleve/etc) can reduce pain & inflammation. If develop fever &/or purulent nasal discharge, see pcp for further evaluation.
